In order to process your application smoothly and efficiently, we would appreciate it if you would follow the following instructions:

Please obtain the following items:

• Form 138/SF9 Senior high school card (Form 137-A will be requested by BTC from your high school)

• One copy of original birth certificate from Philippine Statistics Authority (PSA).
  

• One copy of recent 2×2 ID picture, colored and in white background

• One copy of recent 1×1 ID picture, colored and in white background

• Transfer Credential Certificate (if you have attended college)

• Transcript of Records for evaluation (if you have attended college)

• Medical Certificate
• X-ray (Chest) result
• CBC
• Drug test result

Please have the appropriate people fill out the following forms:

• Form 2a Three Confidential Character Reference Questionnaires (to be filled out by your pastor and two other people who are not relatives). https://forms.gle/jsKpfGwQskCc4RmC9

• Form 3a Letter of Recommendation from an Evangelical Pastor (to be filled out by your pastor). https://forms.gle/Xw54mqSfDwUVooGP6

• Form 4a Letter of Recommendation from an Evangelical Church (to be filled out by your church board). https://forms.gle/PRhd6cAjKs8rvBTAA
